Side Letter Request Form

Please submit this form at least 8 weeks before your event. Early submission gives us time to plan well and extend invitations to musicians fairly.

• 8 weeks before: Submit this form with all of the information requested below. The VP and Operations team will determine potential programs and confirm that musician invitations are distributed equitably under Side Letter 1.
• 7 weeks before: Ops team will arrange any site visit and answer operational questions.
• 6 weeks before: The Operations Coordinator extends invitations to musicians.
• 4 weeks before: All details, including program, personnel, and operations, are finalized and shared with everyone involved.
